I have created a new form using ChronoForms. I have started working on a form, and it can be viewed at:

The idea to start research on this, was based an elance job that basically stated:

Basically this is a petition form that I want to be able to reuse for other petitions if needed.

the form should collect data,
save it to a data base online
and after the person signs the petition by filling out the form...
when they press submit I want the petition to automatically be sent to preconfigured emails that I choose (such as the mayor)

The admin section should allow me to program up to 50 emails at one time allowing the form to be sent using the email address provided by the petitioner (i may want to send to all city council)

The admin section should allow me to display fields and hide fields to require fields be mandatory or not.

admin section should allow me to change the test of the petition and add pictures and imbed youtube into the petition description.

admin section should allow me to see the database which collects all info and also keeps track of how many emails where sent to whom by whom. (the admin section would allow a function on the form that when checked the emails will be sent.) the defult shold always be checked.

the form will not collect more then the following fields if set to do so in admin:

mr/mrs
full name
first name
last name
Title
adress
city/state
zip
Your Location
Telephone
email address
comments
send emails (yes no)
